Enrollment

To elect coverage, you must complete an enrollment form within 31 days of the date you are first eligible.

If you do not enroll within 31 days, you may need to submit evidence of good health to elect State Group Life Insurance or Life and Accidental Death & Dismemberment (AD&D) coverage during a later open enrollment period.
