The School District’s Life and AD&D Insurance Plan
The Life and Accidental Death and Dismemberment (AD&D) Insurance Plan provides valuable benefits for you and your family if you become seriously injured or die while you are insured.
One and one-half times basic annual earnings, to a maximum of $250,000.
Contribution Requirements
Benefits funded by school district stipend. In the event the premium due for this coverage exceeds the stipend, you may be responsible to contribute the difference.
Benefit Reductions Due to Age

| 70 | 65% of pre-age 70 amount |
| 75 | 40% of pre-age 70 amount |
| 80+ | 25% of pre-age 70 amount |
Exclusions/Limitations
Coverage may be continued under the Military Services Leave provision of the Policy for twelve weeks if you go on active duty in the military. However, you will not be covered for a loss that is caused by or arises from such military service, including but not limited to war or act of war (whether declared or undeclared).
For a comprehensive list of exclusions and limitations, please refer to the Certificate of Insurance, which you can find on your school district’s website. The Certificate also provides all requirements necessary to receive a benefit.
