Your School District’s Vision Plan

The Vision Care Plan is administered by Vision Benefits of America (VBA). If you enroll in the plan, you may choose to receive care from a VBA participating provider and receive a higher level of benefits. Alternatively, you may choose to receive care from a non-participating provider and have treatment reimbursed at a reduced level.

Dependents are eligible to participate in this plan provided they are under age 26.

How the Vision Care Plan Works
Each time you or a covered spouse or dependent needs vision care, you must verify that you are eligible for services. Log into the VBA Member Portal to confirm eligibility for services.

Let your provider know that you have VBA.  The provider will submit all claims for covered benefits directly to VBA.  You may be responsible for additional out-of-pocket expenses.

 

REMINDER: Your VBA customer ID # is your state of DE employee ID # proceeded by three zeros.  Provide this # to your service provider.  Appoquinimink School District does not provide social security numbers to VBA.
