Eligibility

Eligibility Under the Appoquinimink School District Plans
If you are a full-time employee and work 10 or more hours a week, you are eligible for coverage under these plans.

• For dental and vision care benefits, you are eligible on the first of the month on or after your date of hire.
• For life and accidental death & dismemberment (AD&D) and long-term disability (LTD) benefits, you are eligible on your date of hire.

If you are absent from work because of an illness or injury on the day your insurance becomes effective, coverage will not begin until the day you return to work. For LTD coverage, pre-existing condition exclusions may apply.

Eligibility Under the State of Delaware Plans
You are eligible for coverage under the State plans if you belong to any of the following eligible categories:

• Perm. FT – Permanent full time 37.5 hours or more per week or 130 or more hours per month
• Perm. PT 30 hrs./wk. – Permanent part time 30 or more hours per week
• Perm. PT – Permanent part time 15 or more hours per week
• Perm. PT 50 hrs./mo – Permanent part time 50 or more hours per month or earning $200 per month for nine out of 12 consecutive months
• Lmt. Term – Appointment to a position that is not of a continuing nature, but is projected to exceed a period of ninety (90) days
