Cigna Dental – PPO Advantage Plan, Group 3340872
The dental annual maximum is calculated from July 1 through June 30 of the plan year.

• Level 1 (Low Option)--$1500 annual maximum per person, major procedures covered at 50%; $1000 lifetime maximum for orthodontics.
• Level 2 (High Option)--$2000 annual maximum per person, major procedures covered at 80%; $2000 lifetime maximum for orthodontics.

DENTAL CLAIMS PRIOR TO 06/30/2017: For any dental claims with dates of service prior to 06/30/2017, please contact MetLife directly at 1-888-303-1113. (The NCCVT/MetLife dental plan expires on 06/30/2017).

Your School District's Dental Plan

Routine, professional dental care is an important part of your family's health care. The School District's Dental Plan offers two choices of dental coverage through Cigna. Both cover a variety of preventive, basic, and major services.

These choices allow you to choose a plan that best meets your family's needs. The plan options differ in the level of coverage they provide and the amount you pay for each option.
