You can continue coverage for up to 18 months by paying the full cost plus 2%
Vision Care
You can continue coverage for up to 18 months by paying the full cost plus 2%
Life and Accidental Death & Dismemberment
Your life and AD&D benefits end. Life insurance benefits may be converted to an individual policy within 31 days of the date you end employment
Long-Term Disability
Your LTD coverage ends
