You Become Disabled

Dental Insurance
You can continue coverage for up to18 months by paying the full cost plus 2% administrative fee.

Vision Insurance
You can continue coverage for up to18 months by paying the full cost plus 2% administrative fee.

Life and Accidental Death & Dismemberment
Your life insurance coverage may continue at no cost to you after you have been disabled for nine months, provided you are under age 70 and became disabled prior to age 60. AD&D coverage is discontinued.

Long-Term Disability
Depending on the level of coverage you choose, you may begin to receive benefits after you have been totally disabled for 90 or 182 consecutive days.
