When Coverage Ends
Your coverage generally ends on the earliest of the:
- Last day of the month in which your employment ends
- Date you are no longer eligible for benefits
- Last day of the period for which you have made the required contributions
- Date coverage is cancelled
Coverage for your family generally ends when your coverage ends or when a dependent is no longer eligible.
Continuation Of Coverage
In certain circumstances, you may continue vision care coverage for yourself and for your family when your coverage ends.
Coordination Of Benefits
If you have vision care coverage through the State or another source, and have coverage through the School District’s plan, you need to understand how both plans compare to each other in order to minimize the amount you pay. Appealing A Claim
For vision care benefits, if your claim is denied in whole or part, you will be notified in writing. The notification will include the reasons for the denial and any additional information required if you want to appeal. Learn when and how to appeal a claim.
