When Coverage Ends

Your coverage generally ends on the earliest of the:

• Last day of the month in which your employment ends
• Date you are no longer eligible for benefits
• Last day of the period for which you have made the required contributions
• Date coverage is cancelled

Coverage for your family generally ends when your coverage ends or when a dependent is no longer eligible.

Continuation Of Coverage

In certain circumstances, you may be eligible to continue dental coverage for yourself and your family when your coverage ends. Please contact the Benefits Department for further information.
