Other Important Information
Continuation of Coverage (COBRA)

Continuation coverage is available to you and your covered family members if coverage under the School District's dental, vision or prescription drug plans ends because of one of the qualifying events described below. To continue coverage, you or your covered family members must apply and pay for continuation coverage before the deadline for payment.

If you have any questions about COBRA or its application, please contact the Employee Benefits Office. All notices described here should be addressed to the School District's Employee Benefits Office. Also, if you have changed your marital status, or you or your spouse have changed addresses, or a dependent ceases to be a dependent eligible for coverage under the terms of the plan, you must notify the Employee Benefits Office as soon as you become aware of the event.
