You End Employment

The chart below illustrates what happens to your benefits when you end employment:

Plan	What You Need To Do
Dental	You can continue coverage for up to18 months by paying the full cost plus 2%.
Vision Care	You can continue coverage for up to18 months by paying the full cost plus 2%.
Life and Accidental Death & Dismemberment	Your life and AD&D benefits end. Life insurance benefits may be converted to an individual policy within 31 days of the date you end employment.
Long-Term Disability	Your LTD coverage ends

What Happens

	You Are Hired
	You Get Married or Have a Child
	You Become Disabled

	You Die While Employed
