You Become Disabled

The chart below illustrates what happens to your benefits when you become disabled:

Plan What You Need To Do

Dental

You can continue coverage for up to18 months by paying the full cost plus 2%.

Vision Care

You can continue coverage for up to18 months by paying the full cost plus 2%.

Life and Accidental Death & Dismemberment

Your life insurance coverage may continue at no cost to you after you have been disabled for six months, provided you are under age 70. AD&D coverage is discontinued.

Long-Term Disability

Depending on the level of coverage you choose, you may begin to receive benefits after you have been totally disabled for 90, 180 or 360 consecutive days.
